Community Health and Wellness

Brigham and Women's Faulkner Hospital (BWFH) strives to attain excellence in patient care services, provided in a learning environment with dignity, compassion and respect. We are dedicated to improving the health and well-being of residents from the areas we serve through our award winning Community Health and Wellness Program. The program serves over 60,000 men, women, children and elderly each year through a wide variety of preventive health education, free health screenings, school partnerships and community outreach.

We have long recognized our responsibility to not only treat illness, but to help prevent disease and the factors that precipitate disease. Through the numerous health partnerships and initiatives that have developed over the years, BWFH works with neighbors to identify and address important community needs and concerns.

Based on the MA Attorney General's guidelines, the goals in the Community Health and Wellness plan are based on an analysis of public health data and needs, community input and collaborative partnerships. 2022 priorities have a primary focus on implementing strategies that will achieve racial and ethnic health equity, including:

  • Financial Stability and Mobility
  • Housing
  • Mental and Behavioral Health
  • Chronic Disease and Healthy Living
  • Access (food, transportation, healthcare, digital, language)

2022 CHNA and CHIP

Every three years, we complete a community health assets and needs assessment (CHNA) and community health implementation plan (CHIP) focused on our priority communities. Find us in the community

BWFH's Community Health and Wellness team is out in the community each Friday in Mass General Brigham's Community Van, providing a variety of free services, including masks and hand sanitizer, COVID vaccine information, blood pressure checks, food bags and other personal care items as available and connection to services (housing, food, utilities, legal, behavioral health, safety, healthcare access).
