Diabetes Program

The Brigham and Women’s Diabetes Program at Brigham and Women’s Faulkner Hospital provides an individualized, patient-centered approach to diabetes management. Our aim is to help our patients achieve their diabetes goals and to empower patients, as well as their families, to become knowledgeable, active participants in their healthcare.

The Brigham and Women’s Diabetes Program provides a full range of diabetes services, including specialized outpatient medical management and comprehensive diabetes self-management education. Our clinical staff maintains close communication with our patients’ primary care physicians to ensure that care is well coordinated. Our team-based care includes a close working relationship with collaborating dietitians, clinical pharmacists and other specialists within the Brigham Health system who provide care for patients with diabetes.

At the Brigham and Women’s Diabetes Program at Brigham and Women’s Faulkner Hospital, our experts specialize in:

  • Individual and group education
  • Pump and Continuous Glucose Monitoring (CGM)
  • Diabetes in pregnancy
