Patients of Brigham and Women's Family Care Associates at Brigham and Women's Faulkner Hospital can request copies of and that amendments be made to their medical records by submitting forms to the Partners HealthCare Medical Records Department.

If you have an urgent need of copies of your medical record or have questions regarding your request, please contact 617-983-7960 (Monday through Friday, 9 am to 4 pm excluding holidays).
Radiology Films and Hospital Bills
Brigham and Women's Faulkner Hospital's Health Information Management Department does NOT provide radiology films or copies of hospital bills. Such requests should be directed to the appropriate department.

- For copies of hospital bills, call Billing Customer Service at 617-726-3884.
If you are requesting copies for purposes of continued care, please state this in your request and an abstract will be provided at no charge. Otherwise, a fee may be associated with your request.
