Emmly Fidelia/Nursing Support Staff Award Nomination Form

Established in 2020 in honor of Emmly Fidelia. Given to a member of the Department of Nursing’s support staff, including PCAs, USTs, MHWs and Secretaries, in recognition of their commitment to the units and staff and their integral role in the delivery of care for our patients.

Nomination Instructions

Nominations may only be submitted by BWFH employees. If your submission does not list an email address ending in @bwh.harvard.edu, @partners.org or @mgb.org, the nomination will not be considered.

Please provide as much detail as possible. It is your words that the committee will consider when narrowing down the nominations, so try to be as descriptive as possible when explaining why your nominee deserves this award!
