Senior Manager of Care Continuum Management hired at BWFH
Leslie Lailer, MM, BSN, RN, CCM, recently assumed the role of Senior Manager of Care Continuum Management at Brigham and Women’s Faulkner Hospital where she will oversee the case managers and the inpatient medical-surgical and addiction social workers and coordinate our response to the medical and psychosocial needs of these patients.
Lailer earned her bachelor’s degree in nursing from the American International College and her master’s degree in management from Cambridge College. In addition to being a registered nurse for nearly 20 years, she is a certified case manager. She comes to us from Brigham and Women’s Hospital where she was most recently Assistant Nurse Director of Care Continuum Management. Prior to that, she served as Director of Care Management at Neighborhood Health Plan.
Transitioning from the bedside to case management proved to be a fulfilling career move for Lailer. “As a nurse, I used to worry what patients were going home to. That concern led me to case management,” she says. “Case managers role is care coordination, care transitions and advocacy. Hospital case managers ensure patients have a safe discharge plan and that they can access the services they need in the community to be successful in managing their illness.”
While she is still getting to know BWFH, Lailer says she is pleased with her decision to take on this new role. “The team is made up of talented and experienced care managers, and they are fun to work with,” she says. “BWFH is a jewel, offering exceptional care and services without exception.”
When she is not at work, Lailer enjoys socializing with family and friends. And stays active swimming and boating in the summer and skiing and snowshoeing in the winter.
