Brigham and Women's Faulkner Hospital (BWFH) strives to attain excellence in patient care services, provided in a learning environment with dignity, compassion and respect. We are dedicated to improving the health and well-being of residents from the areas we serve through our award winning Community Health and Wellness Program. The program serves over 60,000 men, women, children and elderly each year through a wide variety of preventive health education, free health screenings, school partnerships and community outreach.
We have long recognized our responsibility to not only treat illness, but to help prevent disease and the factors that precipitate disease. Through the numerous health partnerships and initiatives that have developed over the years, BWFH works with neighbors to identify and address important community needs and concerns.
Community Health Needs Assessment
A Community Health Needs Assessment (CHNA) is a comprehensive analysis of the health status, needs, and resources within a specific population. Required every three years under the Affordable Care Act (ACA) and the Massachusetts Attorney General’s guidelines, the CHNA helps healthcare institutions and community partners identify the most pressing social, economic, and health issues in the community and develop targeted strategies to address the community's health needs.
For the first time in 2025, Mass General Brigham created a single unified CHNA report to reflect the collective work of two CHNA collaboratives, bringing together the voices of Boston neighborhoods and the North Suffolk communities of Chelsea, Revere, and Winthrop. Under Mass General Brigham's leadership, three Boston hospitals—Massachusetts General Hospital, Brigham and Women’s Hospital and Brigham and Women’s Faulkner Hospital—collaborated to produce one comprehensive report for the region.
