How to make an appointment

Patients: Call the toll-free customer service number on your health insurance card to see if they cover nutrition counseling. Our National Provider ID (NPI) is 1578677811. Medicare will cover only for diagnoses of diabetes and non-dialysis renal disease; however a secondary insurance may cover the cost for other medical conditions. If your insurance covers, call the Clinic directly to schedule an appointment.

Providers: Submit a referral through EPIC to the BWFH Nutrition Clinic (ID 4750). If you do not have access to EPIC, email or call our clinic at 617-983-4455 to request an appointment for your patient. We will contact all patients directly to set up an appointment if their insurance covers.
