DAISY Team Award Nomination Form

The BWFH DAISY Team Award recognizes a nursing team that has made a profound difference in the lives of their patients and family members. The team demonstrates collaboration, using clinical expertise and compassionate care to attain better patient and family outcomes. More information on the award can be found here.

BWFH DAISY Teams:

• Are led by a direct-care nurse, but can include other disciplines.
• Maintain BWFH’s highest standards of care.
• Are role models of teamwork and collaborate to attain better patient and family outcomes.
• Demonstrate clinical excellence.
• Go above and beyond to meet the needs of patients and their families.
• Include 2 or more individuals.

*The Team’s work, including outcomes, need to be included in the nomination.

DAISY Team Awards are presented once per year in the fall. Your nomination will be considered for the next presentation.
