Collaborative Culture of Safety

For the past several years, Brigham and Women’s Faulkner Hospital has been committed to fostering a Collaborative Culture of Safety, formerly referred to as a “Just Culture.” In a Collaborative Culture of Safety, organizations are responsible for building safe systems and employees are responsible for the quality of their choices. Recently, Paul LeSage from SG Collaborative Solutions was onsite to offer his introductory Collaborative Culture of Safety training as well as several advanced trainings to help continue the conversation around the Collaborative Culture of Safety here at BWFH.

“As we continue to reinforce our culture of safety, it is important that we ensure ongoing education regarding our high reliability methodology to address system risks in the hospital and promote patient safety,” says Christi Barney, MSN, RN, Executive Director of Patient Safety, Quality, Risk Management, Infection Control, CDI and Clinical Compliance.

The introductory Collaborative Culture of Safety training was attended by a wide range of BWFH’s staff including physicians, managers, supervisors, clinical leaders and directors. The all-day class provided a strong foundational understanding of the Collaborative Culture of Safety concept.

The next day, LeSage was back to teach a course on bias as well as his 101 and 201 courses to staff who have already completed the introductory training.

In bias training, participants learned about different types of cognitive bias that can affect our analysis of system issues. LeSage says, “It’s important to understand that you have a bias so you can manage your way around it.” The ultimate goal of the training was to improve participants’ investigation skills when looking into safety issues and help them to develop high reliability practices and a Collaborative Culture of Safety in their departments.

Both the 101 and 201 trainings were advanced trainings for those who have participated in Collaborative Case Reviews in the past. The 101 training was designed to refresh participants use of the analysis algorithm with all new cases of increasing complexity. The 201 training offered even more new cases for participants to explore in an effort to reinforce their understanding of our Collaborative Culture of Safety and the algorithm methodology.
